Sports Physical

A Sports Physical from a licensed physician is required every other year. Please bring the below form to your physician’s visit.  Alternate year physical cards are now signed on the back of the Activities Eligibility Form. If you are in need of a sports physical. More information about sports physicals can be found here.

Bellin HIPAA Form

Bellin HIPAA form is signed once every year during an athlete’s high school career.  By signing this form, the Bellin Sports Trainer can legally discuss an athlete’s medical condition with his/her coach. This form is also available in Spanish and Hmong. This is included on the online registration

The online registration for any High School Athletics can be found in rSchool.

The Activities Director will notify the coach of student eligibility.

If your student will be taking alternative transportation home from an away meet/game, you will need to complete the Transportation Waiver Form prior to the event. It needs to be signed by the Athletic Director - please plan accordingly to ensure there is time to get the form reviewed before your child leaves for the event.
